AI is already being used across your organisation. The problem is not any single decision. It is that nobody has compared them.

  • AI is already everywhere - Tools are being used in more places than anyone could list from memory.

    Decisions are inconsistent - Different teams make different risk decisions about similar work.

    Evidence arrives late - Controls and evidence are reconstructed when somebody asks.

    Ownership is unclear - “Who owns the first decision?” still gets a pause.

    The short version: AI ideas enter without consistent risk treatment.

    • Risk must be sized before controls are chosen.

    • Controls must be owned and operable, not merely documented.

    • Governance must remain alive after launch.

    • Evidence must be designed rather than reconstructed.

    • Incidents require declared decision and response routes.

    • The six parts form a dependent system.
